Understanding Health Care Resources

Social work services are provided at no charge to assist patients and their families with nursing home placement, financial assistance information, transportation needs, home care, durable medical equipment, meals, lodging, crisis intervention, and supportive counseling to meet the psychosocial needs of each patient.

The staff provides social work services to patients of Memorial Hospital, Memorial Home Care, Family Palliative Care, Skilled Caring Center, Memorial Health Family Medicine, Behavioral Health Center, and Memorial Counseling Center. Emergency psychiatric assessments and evaluations are also available through the Emergency Department.
