It is important to the workforce at Memorial Hospital and Health Care Center that all our patients and families are comfortable with and confident of the care we provide. Should an issue arise regarding the quality of patient care, hospital administration encourages you to bring your concerns to the department director by calling the hospital at 812-996-2345 and asking for the director of the department in which you received care.

If you feel the director’s response is inadequate, please contact the patient experience officer at 812-996-8513 or the hospital administrator at 812-996-2345.

Since Memorial Hospital is accredited by The Joint Commission, patients also have the option to file issues of concern with them. Please summarize the issues in one to two pages and include the name, street address, city, and state of the health care organization.
