Patient & Visitor Screening Policy

To keep our environment safe, we need to know if patients or visitors have been exposed to an infection that could be passed on to others.


In the past 72 hours have you:

Had a fever of 100.0 or higher?

Developed any new symptoms of sore throat, cough, congestion, shortness of breath, or difficulty breathing?

Experienced new onset of chills, muscle pain, body aches, headache, feeling tired, or loss of taste or smell?

Have you:

Tested positive, been diagnosed with COVID-19, or been in isolation the past 10 days?

Or been told to quarantine due to exposure to someone with COVID-19?

If you're a patient and answered YES to any of the questions above, please enter and notify someone right away when you arrive at your destination.

If you're a visitor and yo answered YES to any of the above questions, please return to visit when you are able to answer NO to all of the above questions.
