What screenings will be included?

Blood Pressure, Height & Weight, Body Mass Index (BMI), Waist Circumference, Cholesterol, HDL, LDL, Cholesterol Ratio, Triglycerides, Blood Glucose, Chemistry Panel lab test, and Tobacco status. Needle draw (venipuncture) is used to obtain blood for Lipid Profile and Chemistry tests. An optional PSA screening (blood test) will also be available to men 50 and older or those at a high risk for prostate cancer. An optional colon cancer take-home screening kit will also be available. Colon screening is recommended annually for adults 50 and older. Additional optional blood tests will include a Thyroid-Stimulating Hormone (TSH) test, Complete Blood Count (CBC) test, and Hemoglobin A1C test.

Do I need to fast?

Yes—12 hours. Take medication as prescribed. (We will schedule individuals with diabetes early if requested.) Drinking water is permitted and highly encouraged.

Why do screenings?

¨ To keep you informed of your health status.

¨ To encourage you to practice healthy habits.

¨ To monitor improved health from year to year.

How long does it take?

About 15 minutes. Appointments will be made every 10 minutes.

What does it cost?

The cost of the basic screening will be paid by the company for all employees and insured spouses. Non-insured spouses may participate in basic screening for $45. Company will also pay for PSA for men 50+. Men younger than 55 wishing to take the PSA blood test will need to pay an additional $22. Company will pay for Colon cancer kits for employees and insured spouses 50+. Participants <50 and uninsured spouses, cost for Colon cancer kits are an additional $9. Company pays for the CBC and A1C blood test for all employees and insured spouses. The uninsured spouse cost for CBC blood tests is $14 and the A1C blood test is $20. TSH blood test is an additional $14 for all participants. The fees for these additional tests will be collected the day of the screening.

Are my screening results confidential?

Yes, only you and the screening staff at Memorial Hospital and Health Care Center will know the results. We encourage you to share the results with your health care provider.

When do I get my results?

A Health Educator will bring your confidential results to the worksite on:

Monday, June 10th 10:00 a.m.—10:30 a.m. EST 09:00 a.m.—09:30 a.m. CST

Tuesday, June 11th 01:00 p.m.—01:30 p.m. EST 12:00 p.m.—12:30 p.m. CST

There will also be an informative PowerPoint presentation to help explain your results. The Health Educator will be available after presentation for any questions you choose to ask in private.
