Event Details

This health screening is available for all MHHCC employees, regardless of your insurance status, as well as their spouses. It will be located in the Mary Potter Classroom. You will need to fast for at least 12 hours, however drinking lots of water is encouraged.  Please take all medications as needed.  Make sure to come to your scheduled appointment on time.

•Screening result sessions will now be done via a Symplr video. This is required in order to receive your health insurance premium reduction for the following year, even if you meet all 5 standards.

•This will replace meeting with a health educator and/or your PCP, however you are still encouraged to do so.

• If you complete your health screening in March, the Symplr video will be assigned to you at the beginning of April, and you will have until May 1 to complete the assignment. If you complete your health screening in August, the Symplr video will be assigned to you at the beginning of September, and you will have until October 1 to complete the assignment.

• If you complete your health screening with your PCP, you will still be required to do the Symplr assignment to receive your health insurance premium reduction in the following year.
