• Screening: $45.00

• This screening includes height, weight, waist circumference, body mass index (BMI), blood pressure, lipid profile (total cholesterol, HDL, LDL, triglycerides, cholesterol ratio), chemistry panel tests, glucose, and tobacco status.

• An optional PSA screening (blood test) will also be available to men 50 and older or those at a high risk for prostate cancer. An optional TSH, an optional CBC, an optional A1C and an optional colon cancer take-home screening kit will also be available. Colon screening is recommended annually for adults 50 and older.

Drink plenty of water the day/night before the screening and morning of screening.

• · FAST for 12 hours. Do not eat or drink anything, except water, 12 hours prior to your Health Screening appointment time.

• · If you take medication or have a medical condition, please consult with your doctor before fasting 12 hours. If you have a medical condition that requires you to have an early morning appointment time, please let us know!

• · Take ALL prescribed medications as directed by your doctor. Take the prescribed amount at the time your doctor has recommended.

• Online registration will be unavailable 24 hours prior to the start time of Jasper Chamber Health Screening.
