· Blood Pressure

· Height and Weight

· BMI (Body Mass Index)

· Waist Circumference

· Tobacco Status

· Blood Glucose (blood draw)

· Blood Cholesterol (Total Cholesterol, HDL, LDL, Triglycerides, Cholesterol Ratio)

· Chemistry panel

· Colon Cancer detection take-home kits, optional for those over age 50

· PSA (blood draw), optional for men 50 and older or those with a risk for prostate cancer

· TSH (Thyroid-Stimulating Hormone—test for metabolism)

· CBC (Complete Blood Count—test for blood disorders, infections, and anemias)

· A1C (Glycated Hemoglobin—test for diabetes)

COST:

Basic Screening: Free for all Employees; $45 for Contract Employees

PSA blood test: Free for all Employees; $22 for Contract Employees

Colon Cancer detection kits: Free for all Employees; $9 for Contract Employees

TSH: $14 (optional) for all

CBC: $14 (optional) for all

A1C: $20 (optional) for all
