What screenings will be included?
- Height & Weight, Body Mass Index (BMI), Waist Circumference, Cholesterol, HDL, LDL, Triglycerides, Cholesterol Ratio; Blood Pressure; Blood Glucose; Chemistry Panel lab test, and Tobacco status. Needle draw (venipuncture) is used to obtain blood for Cholesterol and Chemistry tests. An optional PSA screening (blood test) will also be available to men 50 and older or those at a high risk for prostate cancer. An optional TSH, an optional CBC, an optional A1C and an optional colon cancer take-home screening kit will also be available. Colon screening is recommended annually for adults 50 and older.

- 800 W 9th St. Jasper, IN 47546
- Directions: Screening is in the Mary Potter Room. Go through the front door to be screened and tell them you are looking for the Mary Potter Room. If you go past the outpatient lab, you will turn left down the next hallway and the room will be on your left.
Do I need to fast?
- Yes—12 hours. Take medication as prescribed. (We will schedule individuals with diabetes early if requested.)
Drinking water is permitted.

- To keep you informed of your health status.
- To encourage you to practice healthy habits.
- To monitor improved health from year to year.
How long does it take?
- About 20 minutes. Appointments will be made every 15 minutes.

- Basic Screening: $40
- PSA blood test: $21 (optional blood draw for men)
- Colon Cancer detection kits: $9 (optional)
- CBC: $13 (optional blood draw)
- TSH: $13 (optional blood draw)
- A1C: $20 (optional blood draw)
