What screenings will be included?
- Height & Weight, Body Mass Index (BMI), Waist Circumference, Cholesterol, HDL, LDL, Triglycerides, Cholesterol Ratio; Blood Pressure; Blood Glucose; Chemistry Panel lab test, and Tobacco status. Needle draw (venipuncture) is used to obtain blood for Cholesterol and Chemistry tests.
- An optional PSA screening (blood test) will also be available to men 50 and older or those at a high risk for prostate cancer.
- An optional colon cancer take-home screening kit will also be available. Colon screening is recommended annually for adults 50 and older.
- An optional CBC will be available.
- An optional TSH will be available.
- An optional A1C will be available.
- A personal health risk assessment will be provided for you on-line or in a paper version.

Do I need to fast?
- Yes—12 hours. Take medication as prescribed. (We will schedule individuals with diabetes early if requested.)
Drinking water is encouraged and permitted.

How long does it take?
- About 20 minutes. Appointments will be made every 15 minutes.

Basic Screening: $39 - (Free to insured employees)
PSA for men: $20 - (Free to insured employees)
Colon Cancer Detection Kit: $8 - (Free to insured employees)
Health Risk Assessment: $18 or $20 - (Free to insured employees)
Cash, checks, or credit cards are acceptable at the screening. Make checks payable to Memorial Hospital.
