Who is it for?

  • All Insured Employees, Insured Spouses and Non-Insured Employees

What screenings will be included?

  • Height & Weight, Body Mass Index (BMI), Waist Circumference, Cholesterol, HDL, LDL, Triglycerides, Cholesterol Ratio; Blood Pressure; Blood Glucose; Chemistry Panel lab test, and Tobacco status. Needle draw (venipuncture) is used to obtain blood for Cholesterol and Chemistry tests. An optional PSA screening (blood test) will also be available to men 50 and older or those at a high risk for prostate cancer. An optional colon cancer take-home screening kit will also be available. Colon screening is recommended annually for adults 50 and older. Optional TSH, CBC, and Hgb A1C blood tests will also be available.

  • January 29, 2020, 7:00-8:00 a.m.—Celestine Elementary
  • January 30, 2020. 7:00-8:00 a.m.—NE Dubois Middle School

Do I need to fast?

  • Yes, fasting is required to go through the screening, at least 12 hours. Take medication as prescribed. (We will schedule individuals with diabetes early if requested.) Drinking water is permitted.

What does it cost?

  • The cost of the basic screening will be paid by the company for all insured employees and insured spouses. All men wishing to take the PSA blood test will need to pay an additional $20.00 and the colon cancer kits are an additional $8.00. The optional TSH and CBC blood tests are $12.50 and the optional Hgb A1C blood test is $20. Non-Insured employees will need to pay $39 for the screening. The fees for these additional tests will be collected the day of the screening.
