Healthy Start Communities that C.A.R.E.
Memorial Hospital and Healthcare Center’s Perinatal Services team
utilizes the Health Start Communities that C.A.R.E. program. This HRSA
funded program through IRHA provides support to women before, during and
after pregnancy to help combat Indiana’s high infant mortality rate.
When you sign up, our team will provide you with support, advice, and resources
to help you thrive during your pregnancy and parenthood journey.
Who We Serve
- Women who live in Dubois, Daviess, Martin or Greene counties

Have
any income level
- Are preparing for pregnancy, currently pregnant or have an infant less
than 18 months old
- Partners connected to the enrolled woman can join too!
What Participants will Receive

Regular Check-ins with HSCC Team:
- During your pregnancy to ensure you have all needed supplies, resources
and education prior to infant’s arrival
- Through your baby’s first 18 months of life to ensure you and baby
have resources and support to help you both thrive
- Assistance with finding and connecting to community resources you may need,
such as WIC, transportation services, mental health support, new mother
support and much more.
- Education on various health topics

- Support for your mental health
- Breastfeeding support
- Housing and Food resources
- Bereavement support
- Language Line
- Childbirth Classes

Before and during pregnancy
One-on-one, in person meetings with the Healthy Start team can be scheduled
in your home or in our office. Phone and text communication check-ins
are welcome as well.

- Get tips for adjusting to life with your baby

Learn about managing
postpartum depression

If you need to talk with someone about your feelings or symptoms, make
an appointment by calling (812)996-6384.

If you have feelings of harming yourself or baby call 911 or go to your
closest Emergency Department.
Postpartum Depression is real and we want to be here for you.

Get
breastfeeding support, including one-on-one education with a lactation consultant
- Transportation help to keep up with well child and postpartum appointments.
- Arrange a safety check with your navigator
- Arrange a car seat check with your navigator
- Reach out to your navigator after each well child appointment to receive
incentives.
